service begins with a face
to face visit with an
experienced Registered Nurse
in order to physically
assess and gather vital
information about the client
and family. We view each
patient as a unique
individual with very
specific personal needs and
preferences. Our next
action is to communicate
with the hospital staff and
physician to determine the
type of care that is
appropriate for you or your
family member. Our dynamic
and knowledgeable staff have
visited and ranked numerous
facilities in Palm Beach
County and we have compiled
an ever growing state of the
art database of the highest
quality residencies in the
area . The client and family
are provided recommendations
and preferred choices in
order to make an informed
decision.
Once
a decision has been made by
the client and family
Healthlink Management
contacts the facility or
agency and arranges for the
admission. A Registered
Nurse notifies the Physician
and the plan of care is
developed and implemented.
Once the client is admitted,
a Registered Nurse assumes
the role as the client’s
health care advocate. On
the day of discharge and
transfer the nurse will be
at the hospital and then
will meet the patient at the
home of facility. As a
health care advocate the
nurse makes weekly visits to
the facility or agency to
ensure that quality care is
being provided to the
client. The nurse has an
ongoing communication
network between the family,
physician, and institution.
Regular
visits are made to the area
facilities in order develop
trusting relationships and
make clear the expectations
of Healthlink Management.
Some facilities offer
specialty services and we
are familiar with what is
offered and the quality of
the services. Our
recommendations are derived
from an in depth analysis
that includes state surveys,
certifying agency ratings,
combined with our teams
knowledge and our
proprietary rating system.

Dianne
Nissen is
the founder of
Healthlink
Management, a
health care
consulting and
patient advocacy
firm. She has
been a
registered Nurse
for 30 years and
has chosen to
service patients
and their
families by
guiding and
directing them
through the
health care
system. She
begins by
assessing the
client and
determining the
optimal
arrangement to
meet each ones
unique and
individual
needs.
Dianne
received her
Bachelors degree
in Nursing from
Northeastern
University,
Boston, Mass.
She is near
completion of a
Master degree in
Nursing. Prior
to her
developing
Healthlink
Management
she worked
as a nurse in
acute care
hospitals,
managed an
Alzheimer Unit,
and started a
home health care
agency. Over
the past five
years she has
been a Nurse
Educator. She
has experience
as a nursing
consultant in
the community.
This role helped
her to
appreciate the
complexities of
health care
decisions for
families.
She
is a resident of
Palm Beach,
Florida and
formerly a
resident of
Maine and
Massachusetts.
She has a
daughter who
resides in San
Diego and a son
who lives in
Boston.
Dianne
is familiar with
the health care
network in Palm
Beach County.
As her role of a
patient advocate
she strives to
provide premium
choices within
that network for
her clients.
